Dr. Maynard shares a few recent patient stories where this testing was vital in finding out the root cause for the patients symptoms. Dr. Maynard explains one of the most effective ways to diagnose small fiber neuropathy is through a skin punch biopsy, a quick, minimally invasive procedure performed right in the doctor’s office. This specialized test uses a tiny 3-millimeter instrument—about the size of a pencil eraser—to remove a small sample of skin that can be analyzed for the density of the small nerve fibers responsible for sensing pain and temperature. Before the sample is taken, the area is numbed with a small injection of local anesthetic, so while patients may feel a brief sting from the numbing medicine, the procedure itself is typically well tolerated. No stitches are usually required, and the small biopsy site is simply covered with a bandage and allowed to heal naturally. The skin sample is then sent to a specialized laboratory where it is stained and examined under a microscope to count the tiny nerve endings. If the nerve fiber density is lower than normal, it provides strong evidence of small fiber neuropathy. This test is especially valuable for patients experiencing burning pain, tingling, numbness, or unusual sensations in their feet or legs when traditional nerve conduction studies and EMGs appear normal. A skin punch biopsy can provide the answers many patients have been searching for, allowing physicians to make a more accurate diagnosis and develop an appropriate treatment plan.
